Anyone know if this is safe to use? I know the general rule is nothing that goes between the child and the seat or the child and the straps... but I'm really hoping this is somehow an exception.

It's certainly fine to use with the Sunshine Kids' Radian!   Actually, it's shape is so seat-specific that it wouldn't work well in any other seat anyway.

Although you should never use anything that didn't come with the seat, waterproof pads are generally so thin you could certainly use them without in any way altering the tightness of the harness on the child.  There are several aftermarket products that are quite trim and would work, but honestly, if you have one handy, you can also lay an unstuffed pocket diaper flat in the seat and achieve the same effect.

We have it (in a frontier it fits fine). It's been a good investment. It means we don't have to wash the entire seat cover if there is pee or a spill.

I figure I put ds in a cloth diaper and the in his seat, now he's in underwear with this (super thin) pad so there has always been some extra 'stuff' under his butt.

Awesome. I looked at a couple on amazon, but I figured this one would be the best one since I have a Radian.

I recently stopped putting my son in diapers, and it's been working out great. The accidents are becoming few and far between, but they seem to almost always be in the car. I've had to wash the car seat cover a few times now and it's kind of a pain.
