Need to have my immunization form filled out soon. I don't know what to put...

I am sure this is asked all the time so I am sorry for the repetition.  I am going to drop this off hopefully today and have been looking stuff up online and not finding the answers I need.  

It says:  I, the parent or guardian of the above named child, hereby object to the immunizations listed above for the following reasons:

Sooooooo it would be a personl/philosophical reason.  How specific do I need to be?  I keep seeing forms online to receive an Exemption from Immunizations for Reasons of Conscience Affidavit Form.  Do I need any separate forms?

I would write, "reasons of conscience."

Ohio law says, "A pupil who presents a written statement of the pupil’s parent or guardian in which the parent or guardian declines to have the pupil immunized for reasons of conscience, including religious convictions, is not required to be immunized."

Is there a new form asking for a reason?  If so, I filled out my state's form by saying, "Pursuant to State Code XYZ, which does not require disclosure of grounds for refusal, I am declining the aforementioned vaccines for my child." 

I suppose that I'm growing more callous and bitchy in my parenting years, but I figure that my reasons are not up for discussion, not subject to almighty State approval, and therefore none of the State's business. 

If there's anything else on your state's form that you don't want to sign on to, you have the right to void and initial it.

I think I just put "religious convictions"  to go along with the law...
