Which state form do I need? We live in WI, but school & daycare are in MN

I need to fill out vax forms for DD1 for kindergarten and for DD2 for daycare. We are WI residents, but the girls go to school and daycare in MN. Do I send the MN form or the WI form. Both states have medical, religious, and philosophical exemptions.

I believe the MN one has to be notorized, whereas the WI doesn't? I did a religious exemption last time, although this was a few years ago and I can't remember what the heck I did. Help please.

You would need Minnesota's form--the state where they are going to school and daycare.

You are correct--Minnesota does require the exemption statement to be notarized.
