What to write after checking that I am not vaccinating for "reasons of conscience".?

My DS (child #2) is starting preschool tommorow.  His Dr did not sign the paper I sent him saying that we are not immunizing for medical reasons. (background - my  older daughter recieved some immunizations and had such bad reactions that we stopped immunizing - the dr signed a medical reason for her - and we have not immunized children #2 and #3)  He said to check reasons of conscience and sign it myself.  I am fine with that part.  My question is what to write after the check,  where it asks me to "Please Explain".  Why don't they ask parents why they ARE immunizing their children?  I know with my 1st I immunized because I thought I HAD to!  Now I have so many reasons not to immunize that I don't feel I could possibly fit them on this sheet.  Who is getting this information anyway?

Check you state laws  - sounds like you can file a philsophical exemption. Just write something like "vaccinating is against our personal beliefs". The school keeps all the info to comply with state laws. In theory only the nurse, possible secretary and principle of the school should be privy to this information.
