Exemption Letter or form in MA?

Does anyone have experience with the religious exemption preocess in Massachusetts? I think I'm supposed to write a letter but my information might be a few years old. Is there a form? Or a letter? My DS will be starting at a private preschool.

I don't think there is a form.  Here is the law.

 

"In the absence of an emergency or epidemic of disease declared by the department of public health, no child whose parent or guardian states in writing that vaccination or immunization conflicts with his sincere religious beliefs shall be required to present said physician’s certificate in order to be admitted to school."

When I've submitted my letters I exclude any thing about religious beliefs (since my reasons for delayed/selective vax aren't religious) and have only been called on it once.  I changed it to sincere beliefs which again didn't cut the mustard at this school so I lied, called it religious belief, and was fine.  Don't sweat it!
