New here, high risk, would like some input!

Hi Ladies! I was referred here by a friend. Here is my story:

was on metformin). Once I got pregnant I stopped the metformin and had a normal
pregnancy...until 13 weeks. I woke up one night, ran to the bathroom and opened
my eyes to a pool of blood on the floor, streams down my legs and a bright red
toilet. I immediately called the doctor and went to an urgent care to get an u/s
just to see baby. Baby was totally fine. The next day I went to my OB and had
several u/s and they just couldn't figure out what was going on, but I was told
to stay in bed and if I had anymore gushes of blood or filled a pad in an hour
to call. Once I got home and got settled, I filled a pad in 10 mins. I was sent
back to an ER where they did several u/s but said they found nothing. I
consulted with my OB and she said she found out from my u/s that I have a BU and
there was a hematoma and blood clot the size of the baby in my uterus. The
following week, I
experienced labor-like cramps and was passing blood clots the size of
half-dollars. My OB didn't want me to come in bc I needed to rest and if I was
going to mc there was nothing they could do. Since my clots weren't egg-sized,
they weren't concerned. After that week everything was fine. I was on bedrest
until week 22, when I was only allowed to do moderate activity and rest as well.
At 26 weeks, I woke up to the blood once again, this time I made it mostly to
the toilet. We went in to L&D and they hooked me up and I was contracting every
4 mins, so they knocked me out. I was on everything. After a few days they sent
me home on bedrest once again. 2 weeks later it looked like I was losing my
mucus plug, so I went in and they said everything looked fine, but they wanted
to keep me bc of my blood pressure. In the middle of the night I went into ptl
again, and got put on the mag again. The next day several tests were performed
and that evening they decided
that I needed to be induced due to the fact that I had extremely severe pre-e
and was risking the health of myself and my baby to stay pregnant. At 28w6d I
vaginally delivered my baby girl born at 1 lb 13 oz and 12.5 in long. She did
fairly well and stayed 2 months in the NICU. Today she is a healthy and thriving
15 month old baby girl, and couldn't be happier. I am also 8 weeks pregnant.
This time around, after consultations and research and an HSG to confirm a BU
with no septum, DH and I decided to go for it. I was still nursing DD and had
not had a pp af. But, I magically got pregnant after 3 months of trying! I had
an u/s at 6 wks and 8 wks. My OB gave me a precription for progesterone
suppositories, which I don't think I am going to take, and wants me to start the
prog injections at 16 wks, which I am still debating. Reasons being, I found out
some information from the friend who referred me here. I don't think my OB
really knows as much as I would
like her to know about my uterus. So my questions are: if you have a BU what
have been your experiences? Anyone have bleeding like this? Is it common? If you
got pg again, what restrictions have you had from your dr, ie sex, exercise,
lifting? What is your plan, ie progesterone, cerclage? Do you have an OB and
Perinatal? I am thinking about finding a Peri or asking my OB to refer me to one
for my u/s. I dont know if her u/s has the knowledge to know what to watch.
Also, is anyone from the Houston area that can refer me to anyone? Thanks ladies
and sorry that was so long! I wanted to detail it as much as possible so I can
get some good info back :)

Welcome to MDC!!  wave.gif  You've come to the right place for advice and support.  But I see that you haven't gotten any responses yet.  I don't have any experience to offer you, but wanted to make a few suggestions to try and get you connected with those that do. 

 

First, I would change the title of your post to reflect your exact question.  There are a lot of reasons pregnancy could be high risk, but you want to find those that had a similar issue as you.  So maybe something like, "Bicornuate Uterus, previous hematoma & pre-term labor- how to prevent second time."  Or something like that.

 

Second, I would post your question about care providers in the Houston area in the "Finding Your Tribe" section for Texas.  That's where you'll find locals who can help you out.

 

Third, I would join your due date club/social group to connect more intimately with women going through the same stages of pregnancy at the same time as you.  The DDC's and groups tend to get a lot more consistent traffic so people will answer your questions quicker and even if they don't know the answer, they will lend you support, which can be very helpful.

I don't have the same specific problem as you do, however I am also considered high risk and have seen my OB in conjunction with a peri for each of my subsequent pregnancies.  I definitely think you should talk to your OB about being followed by a perinatologist as well.  Good luck and keep us updated!!

I don't have BU, I have UU, and my suggestion to you would be to join the Mullerian Anomalies Yahoo Group.  That is the biggest source of information on the net for women with MAs, and many, many women there have BU. 

 

But, I'll still give you my 0.02c.  Generally speaking, having BU would not make you high risk, but what happened in your last pregnancy would certainly have an impact on that.  It sounds to me like the main problem with your previous pregnancy was the SCH, which may or may not be related to your BU.  There are studies which have found links between MAs and blood flow anomalies. 

 

Having said that, the Pre E, would be something that would warrant additional follow up.  I don't think that you would be out of line to get a Peri consult, but close monitoring, especially of your blood pressure, from your regular OB might be fine too. 

 

As far as the SCH goes, you can't really "prevent" it.  In most cases, there is no known cause. 

With a SCH, you have blood in the uterus.  Because blood is an irritant to the uterus, it can cause contractions.  That's where the progesterone comes in.  It works to "relax" the contractions, or potential contractions.  Is there are reason that you don't want it?

If you were to develop another SCH, then I would suggest pelvic rest is a good idea.  You don't want to risk irritating the cervix at all.  I'd be keeping to gentle exercise only. 

 

With the cerclage, it doesn't sound like that's necessary for you, unless you had some cervical shortening that you didn't mention? From what I can gather, it was the PTL, and PreE that necessitated the pre term delivery, and not cervical incompetence? 

If there was a problem with shortening, or you are worried, you could ask for regular ultrasounds to check the cervix length... normally from about 16 weeks.
