I wonder if she pushed when she felt like it, rather than doing "purple pushing" like hospitals do? That might explain part of the time.
EDIT: I read her story, and she said that the midwife and assistant were pretty hands off until she was five hours into pushing. At that point they said "we have some suggestions for you." So, who knows. Maybe the baby was malpositioned?
