-20yrs old
-Uncircumcised
-penis won't retract
-very little feeling during sex/mastb
-can't climax during sex, takes hours to climax with mastrubation
-low-income, but has insurance not sure what kind Portland OR area

As an alternative there is a dorsal slit or even just removing the sphincter at the end so that he could keep the majority of his foreskin. Circ should be the very last option that he considers.

You will have read in the CIRP article that the first course of action is the use of a steroid cream (Betamethasone .05) with gentle stretching. If, after a couple of months that does not work, there are many versions of preputioplasty, which involve a (or several) small longitudinal incisions that are then sutured transversly, effectively widening the prepuce.  There is also a procedure developed in Europe, that looks to be less invasive than preputioplasty:  www.biomedcentral.com/1471-2490/8/6  .  The results are quite impressive.   The only reason that this doctor is talking circumcision is that he has never bothered to educate himself about all the less invasive procedures, which I find shameful at best.

If it aint broke, don't fix it. But since it sounds like it is broke, fix it. Don't cut it all off though. Like others have said, there are good alternatives.
