Declining Vax in New York State

I'm posting for my cousin and his girlfriend. They have postponed their daughters appt for an MMR shot several times. They keep rescheduling the shot, but want to avoid it completely.

I live in MI and am allowed a philosophical exemption. It looks like they just have religious or medical for NY. Does anyone know if they require some sort of proof of religious affiliation?
