I saw this on Pinterest, and it does look like it could be comfortable for a sleeping toddler on a long trip, but couldn't iit suffocate or strangle or something bad? Or am I overly imaginative?

I've seen that (I, too, love Pinterest), and it seems very silly.  It would be harmful for very small children, and I can't imagine any child old enough to move their hands would tolerate it for long.

I wouldn't worry so much about suffocation, but more about what might happen in a rear-end crash. I don't even want to think about it!

Yep, rearface your kid as long as possible, by the time they outgrow rearfacing seats they will be older, with more head control and overall less likely to sleep in the car.  Things like this most often seemed to be used by younger toddlers who are already forward facing, rearfacing would solve the problem.
