No Vax in Wisconsin & Public Schools

So far we have not vaccinated our children (We have 2, and 1 on the way). School registration is in 2 weeks for our oldest, who will start 4K. Tell me what I need to know so I can be prepared, I don't want to have pregnancy brain and emotions get the best of me.

My mom said she just had to sign a waiver that the school gave her, but that was 15+ years ago, just want to know if things have changed.

No they haven't! I signed the waiver when my daughter started 4K last year, no problems!

Did you have to do anything else besides fill this out and check the exemption

Do I need to print and have the affidavit from this site notorized -- is that required?

When my kids were in public school (a couple of years ago) we just filled out the form they gave us, stating that they weren't up to date on vax & we understood if there was an outbreak they'd have to be kept home. No extra forms or notorization required :)

We just signed a waiver too. No notorization needed.

The forms that they give you also has a waiver on it that you can mark.  Easy peasy!
