I usually say something like, "Did you not hear me, or did you not understand me?" And then rephrase or speak more loudly as necessary, otherwise I kind of just ignore the question once I've answered it half a dozen times... I'll admit this drives me insane, much more than the constant "why...?" and "what will happen if...?" and incessant talking.
