Where do I get Religious Vaccine Exemption Forms for schools in NY?

My almost 4 yr old daughter will be attending pre-school in the fall and since we've decided not to vaccinate, I'm wondering what forms I need to claim religious exemption in NY. Is there a website I can go to? Do I have to contact the school prior to handing in the forms? Do I need anything else? Any advice would be greatly appreciated, as this is my first time doing this! Thanks Mamas! :)

I would not contact the school prior to handing in the forms, because that would look like you are asking permission.

Here is an attorney that helps with New York exemptions. http://www.patriciafinnattorney.com I have no experience with her. The website just came up when I Googled "10 NYCRR, Section 66-1.3 (d)" which is the applicable New York law.
