Vaccine Dates on Health Form For Vacation Bible School

My son is going to vacation bible school for the first time this summer and they have a health form that asks for the dates of certain vaccines. They don't ask for proof of vaccines....just the dates the child had them. My son hasn't had any. We live in a small town, and I'd like the fact that we don't vaccinate to stay confidential.  Do you think I should put not vaccinated on the forms, up-to-date, or something else?  I hate to lie on the form. I just want it to remain confidential if we do share that we don't vaccinate.

My ds's soccer club requires him to be utd on vaccines, and asks us to list them (no proof required), I just write "see attached exemption" in the space, and include a signed copy of the school vax exemption with his registration package. The club has always accepted this without question. DH was very up front with them in the beginning, if they don't accept the exemption, then they can't have ds play for them.
