Not UC have a midwife, but need info on getting a Birth Certificate in NY

My midwife is a PA midwife so she can't help me with getting a birth certificate. Anyone know how to go about getting a birth certificate in NY when you don't have a hospital birth? I tried searching online to really no available info.
