Sports Physical...... Immunization Trouble????

I am a home schooling mom and up to this point I haven't had to mess with back-to-school/sports physicals.  I have an appointment scheduled for Tuesday at our ped's office because my oldest son will be playing football this year.  Can anyone tell me, will they be asking me if my son is up to date on his vaccines?  I mean, our nurse practitioner knows, but is it part of the checklist that is included on the form I have to turn in??    Do I have to worry about obtaining an immunization waiver??  I hate being hassled about vaccines and want to be prepared.

My children are unvaxed and have had to have school physicals before, but it is never been an issue at the doctor's office (I will only see drs who are supportive of my stance) and they have just signed off on the form, stating that I have chosen not to vaccinate. My DS's soccer club does require the boys to be fully utd on vaccines, but I just provide them with the same exemption as with school, and it has never been an issue. I am in CO so vaccine exemption is a very easy process and includes a philosophical exemption, although we use religious.
