need help with religious exemption in New York

My family recently moved to New York and my children are not vaccinated. We never had any issues in the state that we moved from, but now they are threatening to kick my kids out of school if I can not provide proof of vaccinations. I know about religious exemptions, but I have 2 questions.... Is there a specific form that I need, if so where can I get it? And secondly, what kind of proof is needed in order to claim religious reasons for not getting my children vaccinated?
