religious exemption for immunizations

I have a delimma... several of my families are opting out of immuniziations.  Yet the local health department is refusing to give the link to down load the necessary form.  Does any one have access to the form where these parents can fill in the box for religious exemption?  I need it ASAP. Thanks:}

I am in washington and I am having the same problem finding the link to download a religious exemption form.

In Oregon the form is on this page:

Click on "Oregon Certificate of Immunization Status"-- it's a pdf doc you can bring into school or wherever. There's just a box on the back for parents to say which vaccines they are religiously exempted from-- easy! No one should ask any questions, but you may need to point it out since it's relatively unusual.
