ASD and not vaccinating

I found a NP in Anchorage who is a-o-k with us not vaxing(yay!) but now my problem lies with the school district. Homeschool and private aren't options. I looked on the ASD site and they had the religious exemption form there, and it just said the parents have to be aware of their decision and have a notary sign it. It doesn't say anything about a religious official being the notary, so could we essentially just go get it notarized at the bank or something?

I don't think it even has to be notarized at all.

This is the form I found on ASD for religious exemptions(my NP already said while she agrees/supports our decision, she can't say it's medically necessary for us to not fax :() I just thought it had to be lol.

We had our exemptions notarized at our credit union. Turned them in with each kid's registration info and it was no biggie.

Would love to know your NP! We're still with a mainstream ped, but I'd love to find a more naturally, holistically-minded family practitioner.

Awesome!!! We're not actually religious... so I was worrying about having to find a religious official to notarize for us haha.

This is going to sound horrible, but I forgot her name :-x We only have talked on the phone, but we have our first appointment on Thursday. She's the NP at One Family Birth Center :)!

I live in the Kenai but I used religious exemption just fine. Nurse didnt even bat an eyelash.

It doesn't need to be notarized.  Her name is Heidi!!

Does not require being notarized. There is a space for it on the form, but it is not required.
