Maybe they didn't mention their name here but all names and University info is given on the first page of the survey, as well as phone numbers for not only the researchers but their supervisors at the uni.
Looks legit to me.
I gave a fake name because, even though my son has a relatively common name, I also found it kind of weird too that they ask for the name. But not weird enough to make me doubt its legitimacy/
