Any mamas UC in Wisconsin?

Hi, I am in WI and planning on a UC. This will be my 5th baby. The last two were in Portland Oregon with Midwives so they handled all the paper work. Not sure where to start with that doing it on our own. Thanks!

HI! I had 3 UCs (and one self directed hospital birth) in & near Madison, WI. For the birth certificate all you have to do is call the vital records -- or registrar of deeds, it slips my mind at the moment which -- and ask for the paperwork to file an out of hospital birth.  They can send you the packet which needs to be filled out in, i believe 5 buisness days from the time of birth, and they require a signature from a healthcare professional verifying your identification & basically stating that they confirm the baby is yours (we had our family dr or ped sign for us) If you have any more questions don't hesitate to ask (or PM me ;)

Awesome! Thank you. We are about 30 min SW of Madison. Is this packet something I could have sent to me now?

Mt horeb area?  We are close to one another. I'm in Mazomanie :)  I host a moms brunch every week and a women's circle (discussion group) once a month so if you are interested in connecting with other women locally you are welcome to participate in either/both of those.
 

They should be able to send the packet to you now. I always requested them in advance so i had it on hand ready to be filled out when baby arrived.
