So, it's been a difficult week! My baby hasn't been content and relaxed during the days. However, our night are good. Her main show of fussiness over the past 5 days has been pulling off the nipple and wailing. She doesn't feed enough to get her belly full and go to sleep, so she had a couple of unhappy days when she stayed awake 9 am to 4 pm with no naps. I didn't know the nipple pull off and crying was a symptom of thrush till I read about it somewhere online. I also have had constantly sore nipples but not to the point that they feel like "shards of glass" are poking them.
Two days ago the baby developed a diaper rash which based on online photos looked like a yeast rash. At that point, I acted more quickly and yesterday I got a prescription for nystatin for her rash and all purpose nipple ointment (APNO) for me. I couldn't see any white spots in her mouth and I was hopeful that the APNO could treat anything going on in her mouth. However, I wasn't certain so I brought her into the pediatrician today and the doctor confirmed it is thrush. He said she has just a small spot in her mouth at this point, but that can cause pain and even reduce her sense of taste to the point that she doesn't enjoy milk as much. He also confirmed that the diaper rash was yeast (the nystatin already seems to be working). The doctor prescribed oral nystatin for her mouth and we're picking up that medicine now.
I am glad to have a definitive diagnosis but the pediatrician didn't educate us about thrush. He just said that the meds will clear everything up and I don't need to worry about passing it back and forth between baby and mama again and again (although I've read lots of stories online about this happening).
I have several questions...
- I feel like I might have a minor vaginal yeast infection. Can I use the same nystatin cream I'm putting on her diaper rash for my yeast infection in hopes it works for that too?
- How much should I be concerned about her cloth diapers (cotton prefolds and GMD workhorse cotton fitteds) reinfecting her with the yeast diaper rash? I washed all her diapers yesterday in hot water with a vinegar rinse at the end. Can I start using them again now or should I do additional washes? This webpage on disinfecting cloth diapers for thrush is confusing to me... http://www.pinstripesandpolkadots.com/yeastvscloth.htm. Can I safely use bleach on cotton dipes? Is it necessary? Is it OK for the nystatin diaper rash cream to come into contact with the prefolds and cotton fitteds? Or should I use disposables till this is cleared up?
- Do I really need to throw away the milk I pumped during the thrush outbreak? I haven't saved much so it's no big deal. However, I saw on one website that the baby's immune system should be strong enough to deal with the thrush in the frozen milk in future months.
- What other cautions should I take to get us better and end the thrush cycle? I've seen so many ideas online and frankly don't want to do most of them... such as eliminate dairy from my diet, wash the baby's hands constantly, stop using cloth diapers and switch to disposables until the thrush is gone. According to the pediatrician, I have nothing to worry about -- he thinks the thrush will go away with the medications and I don't have to do anything else. But I have seen many websites with long lists of thrush control protocols, so who should I listen to?
