Anyone had a homebirth recently with the Hackettstown Midwives? A few questions

I'm considering a homebirth with the Hackettstown Midwives, well, I guess they're the Midwives of New Jersey now, but anyway, I know my insurance will cover them at an out of network rate but I'm wondering what the other costs are?  If I understand correctly, there is a $500 fee for their backup doctor then the cost of the homebirth kit, the pool liner (I'm definately doing a waterbirth) and the cost of the homebirth class.  Is that everything they require?  How much is the class, kit and liner?  Thanks in advance!
