Need HELP with vax waiver form for school registration in CALIFORNIA

Hi,

 

I spent the last 2 hours on the phone to solve a problem, no success, hope someone here can help me:

 

We are living in Canada, moving to California (Mountain View) middle of next month. All our children are not vax-ed at all. To register the older two for school, I need to have the PERSONAL BELIEFS EXEMPTION TO REQUIRED IMMUNIZATIONS form signed by an AUTHORIZED HEALTH CARE PRACTITIONER LICENSED IN CALIFORNIA. That can be an M.D., nurse practitioner, physician assistant, naturopathic doctor or credential school nurse. Here is the form:

The clinics/doctors/naturopaths I called this morning all require my children to be patients, so they need to see my children to accept them as patients, and then they can sign the form. Or we can go to a walk-in clinic on the immunization day, I only found some for which that day is Wednesday.

 

Our constraint is that my husband is in the area only next Monday. We want to get the school registration done then, to have school sorted out BEFORE the Christmas break so that our kids have a clean start in the New Year.

 

My question, ask for HELP  to you:

Can you connect me with any practitioner in California who could give me a phone consultation and then sign, scan and send the form to me??

Or can you connect me with any practioner in Redwood City/Palo Alto/Mountain View who could see my husband next Monday and sign the form?

We are happy to pay for the practitioner's service out of pocket.

If they start school before January, you can use the old form, which does not require a health professional's signature.

Interesting!

They will be registered in December but their first day at school will be in January only.

Sufficient to use that old form?

Where does it say that this is the old form, and that one should use the new form?

If you register your kids in December, but they aren't scheduled to start school until January, it is unclear whether you can use the old form. Since it's not clearly stated in the law, you might have a difficult time arguing with the school if they insist on the new form.
