I had an episiotomy with my first, who was 7 lbs 2 oz. It was a hospital birth and the OB didn't give me time to object. The OB stiched me for 20 minutes.

My second was born naturally ... and I did tear but only required a couple stitches. My second was 9.5 lbs and his chest was wider than his head. I believe his head was 14.5 inches and his chest was 15. Bigger than average!

With my third, I tore also and required a couple stitches...and I believe that the only reason I tore was because she came out so fast...I had one HUGE pushing contraction that I couldn't stop, so her head and body birthed amazingly quickly.

I, too, had scar tissue and was very scared. It' wasn't nearly as bad recovery-wise the second time around either.

As far a position goes I think you just have to do what feels right to you. With my HB transfer I delivered flat on my back and tore with all 3 episiotomies to the third degree. With my second birth I also delivered on my back but did not tear at all. I tried delivering in the water but just couldn't find a good push. My MW suggested I walk up the stairs and rest and when I got up there I pushed her out on the bed. She told me later that she thinks my pelvis is shapped funny(due to my previous birth transport due to severe malpresentation) and delivering on my back may help my babies navigate my pelvis better. I am not forced on my back by anyone so I feel comfortable delivering that way next time if that is what happens.
