Retracted himself once, can't now

My DS is 2.5 years old. When he was a little over a year, I saw him retract his foreskin completely - he did it just once. It didn't seem to cause him pain.

He hasn't done it since, and out of curiousity I asked him yesterday if he could, and he pulled it back a tiny bit and then said "no."

Is this abnormal? Is it possible he himself caused some damage from that first instance? If so, would it heal by itself, and should I even worry about it? Should I just make like he'd never retracted himself, and wait until after puberty to worry if he can't do it again?

Unusual? Yes. Abnormal? No.

What happens in young boys is that the penis grows faster than the foreskin and he just happened to catch it at an odd moment in time.

The evidence of the penis and foreskin growing at different rates can be observed in infancy the foreskin extending a substantial distance beyond the glans and at puberty, there is little to none extending beyond the glans. It's kinda of an instance of the penis catching up to the foreskin. You don't need to do anything and neither does he. (Unless he wants to) When the time comes, the non-elastic tissue in the band around the opening will be replaced with elastic tissue and he will be able to retract quite easily.
