Need a link on timelines of disease decline

Does anyone have a link that shows when the various diseases started to decline, specifically in relation to the timelines where vaccines were introduced? I know much of this information is in the Miller book, but I loaned my copy to a friend.

I believe I also saw some information related specifically to polio on the Weston Price site:www.westonaprice.org

If I remember correctly, the incidence and decline of polio coincided nicely with the decline of a certain pesticide that was outlawed in the 50's...
