Another thing that helps with my dd is talking about the future and what is going to happen during the day. We aren't big on routine here, but I do try to discuss everything...I find we have easier transitions if we talk about having dinner, playing a game, having a bath, reading, then bedtime, for example. Usually I don't do tooo far ahead, just a couple things, but every evening we talk about what we are going to do the next day. And each day I always at least try to get out of the house for a walk, since I need the sunshine for my mood, so at least I can say, "And we'll go for a walk."
She loves talking about the next day. She is learning about time and really likes to talk about things happnening at such-and-such o'clock, i.e., "I'm going to get up a 4 o'clock." UMMM! LOL. But she enjoys talking about time even if is still learning what it means (i.e., the future is always "Wednesday," the past is "last night.")
