Help on getting Birth Certificate & SS#

I have not recd my paperwork from my midwife & its been over a month since my son was born. I really need to start the process for the birth certificate & social security number. Hes not being covered under our insurance because of this & the bills are starting to pile up. Is there anyway that I can do this myself without the paperwork from my midwife? How do you do this if it was an unassisted birth?

hey, carrie, you have a beautiful name!

you can get a ss# by filling out the form <a href="http://www.ssa.gov/online/ss-5.html"> here</a>, i believe. i would google "[your state] birth certificate application" and i'm sure you'd find the form on some sort of state website.

DS has a SS# but no birth certificate yet... you have to have a billion forms of ID (ok - so maybe just two ) to get a birth certificate and i haven't gotten copies of what i need. i'm not positive on this, but your insurance should cover your DS without a SS#. we added our DS before we'd done any "official" paperwork and just put a DOB on the insurance app, figuring if they HAD to have a ss# they'd let us know, but if not, then we didn't need to give it to them. we've received documentation that says he's covered, so i would definitely at least try to get him on the insurance.

that said, it didn't take very long (a few weeks?) to get the SS#. HTH!

I think that this varies from state to state. My first dd was born at home, and we did not have our midwife's name on the birth certificate--since a lay midwife is not legally a medical practitioner anyway. Also, midwives are very legally vulnerable here, so we kept her name off official things. We got this certificate after 2 mos, but we got our fourth child's birth certificate after she was 2yo. This time I had better documents from midwife, but still she was not on as the "attendant" (actually she got there after my dd was born anyway!)

We had to have:

Proof that I had been pregnant. This could be photos, statement from a medical professional, any health records including that info. I did have a small amount of prenatal documents both times, and a few photos.

Proof of live birth. I went to a family friend who happened to be a doctor, and she certified that I had given birth to this child. Short note, saying I had given birth and corresponding baby was mine. Other birth I had birth records and photos of me nursing the newborn, etc.

Attendant signature. Or witness if no attendant. So my mom signed and my dh signed for dd4.

I.D.s for me and dh.


Even after 2YEARS, there was NO hassle.

Once you have birth cert, SS# is very simple. You use the birth certificate to get it.

We went to the county health dept and filled out the birth certificate ourselves. It was easy as pie. Our mw is a lay mw so we couldn't use her name and we didn't need any paperwork from her. My dh is listed as the birth attendant.

For the SSC, I went to the local office with the birth cert and a dr note (they wanted immunization record but we didn't immunize). It was pretty simple.
