Exemption form & disclosing prior vaxes?

I am in WI and both my kids are fully vaxed up to now. I am going to file a philosophical exemption for dd#2 from the kindergarten shots and beyond. My question is should I fill in the dates of the previous vaxes she received? I would leave it blank, but we already filled out a form last fall for her preschool listing the dates of her vaxes so I wouldn't want to raise any red flags. Also dd#1 is 8 and when the time comes for other shots they require for older kids I will file an exemption on her too, so the same question applies. I've filled in the dates for her previous shots on the forms in the past. Should I just continue to fill in or leave it blank?

I guess it doesn't matter. You probably won't even show them her vaccination booklet. Just file an exemption form.

No. Don't fill in anything. And don't bring the vaccine record with you.
Just file an exemption form for her now, also.Don't wait until she is due for more shots. This way you have everything in place.

It shows you have decided to stop vaccinating both girls at this time of your life.

Oh good point! I never thought of that but I will for sure do that. So it doesn't really matter that the school or state or whoever has a previous record filled out by me with the prior shot dates? I should just not fill in anything anymore?

I posted your state's exemption form on your other thread. It makes no difference if you put in prior vaccinations . . . in fact they even request it for those seeking exemptions.

It's up to you, but your exemption would not be challenged if you disclosed the prior vaxes. If your state only allowed for a religious exemption, then I would tell you not to disclose it, but your state has the philo.

Oops, I'm mixing up my threads. Now that I see the form again I see I'll just have to check the box for the phil. exemption and it asks to list any prior vaxes in the appropriate boxes on the form. Thanks so much LongIsland for the info!

When dd had her pre-kindergarten physical the nurse at the dr office filled out the physical form and put all dd's previous vax's on it. Here it is required that they have a physical before each school year and the form has the boxes for the vax's to be listed. So there is really no way that I could not turn them in. Lucky enough nothing has been said but she has had all her shots up to the last round she was supposed to get at 4yo Ds's will have his 2-4months shots on it but nothing past that. I wounder if they will give me greif over it since he obviously hasnt had most of them.
