Hmm. I don't think it should be hurting you that quickly! Is your sling too big/little? Is the fabric spread across your shoulder blade and back? I know when I first started wearing dd I was a little sore, but now I can wear her for hours and not feel a thing. Keep messing around until it feels good to you!
I know it is possible to sling two babies at once, but I don't know how. Try going to www.thebabywearer.com and go to the articles/using your babycarrier. You might be able to find out there about wearing two at once. Another good site is www.mamatoto.org.

HTH!

It hurts because the OTSBH doesn't fit you. Your baby's weight should be as close to your center of gravity as possible, and the fabric of the sling should be even, not bunched, and spread out across your back as smoothly as possible... with the heavy padding of the OTSBH that is nearly impossible but not completely. I wore one kid in an OTBSH for 2 years and survived. But an unpadded sling is MUCH more comfortable and it's easier to put twins in too. With an unpadded sling, twins can ride together. You might look at www.mayawrap.com where they have a video online, with a special section of the video directed at wearing twins.

Also, the sling really shouldn't be touching your neck at all. It should be capping the ball of your shoulder. But I agree, it is almost impossible to get a heavily padded, closed-tail sling snug enough to support the baby comfortably.

I almost gave up on my sling, until a post on here taught me that the sling should go OVER your shoulder, not just on top of your shoulder. it should actually go down your arm a wee bit. Now i can carry my 15 lb 3 mo old for up to an hour with no probs!

Yes, make SURE it is on your shoulder, not by your neck. That is crucial. Take a look at the Mei Hip carrier (for example at Peppermint.com) - that is a different type of carrier but that is where the OTSH should hit your shoulder. You will notice a Huge difference!

Yup the sling should be spread well across your shoulder and not on your neck. Also make sur eits spread well on your back or your shoulder will hae pressure points that'll fee less than great as well!
