Appealing a health insurance decision -- how to do it?

I submitted our home birth bill to our insurance company (Medical Mutual) and it was denied, as I expected it would be. I know there are some women who have successfully appealed this, but I honestly don't know where to start. Can anyone help me with this? Thanks!

don't have time to look it up right now, but is ohio a legal state for midwives to attend homebirths? (i'm assuming you worked with a midwife...) can you submit claims for prenatal and postnatal visits to your midwife instead of a homebirth claim? the code for a prenatal is the same regardless of the type of provider. is your midwife licensed? if so, you may have a very good chance of being successful in getting payment for the midwife.
