Our doctor needs to approve us?????

Since we get medical assistance, we got a small list of doctors to choose from and had to pick one. I picked the only one in town. My son needs to have a health form filled out for daycare, so I called to schedule an appointment. The secretary said that we have to fill out a questionairre and the doctor has to approve us before we can be patients. WTH? I've never heard of this. What's the likelihood that we will be rejected because we don't vaccinate? I'll update when I recieve the questionaire in the mail. :

I don't know, but having to apply to be approved as a patient doesn't look like a good sign . Maybe if you submitted the questionaire with a religious exemption it would help.

Absolutely submit a religious exemption form with the questionaire. It wouldn't look so hot if the "good doctor" didn't accept you because of your beliefs.

I've never heard of anything like that either, although I'm not surprised. Something tells me the questionnaire will include questions similar to these:

*Is your child up-to-date with his/her vaccinations?

*If not, why?
___ Prior reaction to one or more vaccines
___ Delayed schedule
___ Opposed to all vaccinations

*If you delay vaccination, how far behind is your child? What are the reasons for delaying (e.g. safety, too many vaccines at once, etc.)?

*Do you anticipate your child being up-to-date with his/her vaccinations prior to school entry?

If there are questions like these on the form, then you should submit a statement similar to a religious exemption for school entry.

Sheesh. I would probably just fill out the stupid health form myself. Sign it "Dr. Smith" or something. It's not like the daycare is going to check it out. They just need the form for their files. When I had to get that kind of form from my doctor she just wrote "WNL" in every blank (like for DS' height/weight/headsize/etc.) and then signed it. I didn't know what "WNL" meant so I asked. Turns out it means "within normal limits."
