Oh gosh, I'm not really sure since my little guy isn't old enough to be worried about that yet but I do remember DD being worried about peeing in her undies when she was about 3. She would get worried when she had to go pee and then would comment about peeing in her pants. Has someone been talking to him about it or maybe teasing him from a time that it did happen? If so that could make him tend to worry that it is happening.

Is he hot? When I'm REALLY hot, sometimes I get sweat dripping down into my underpants. I could imagine it easily happening to a child too. Maybe all he feels is wet and is unsure where it came from.
Are his underwear too tight? We recently had to go up a size in undies -- and the old ones are much tighter and feel uncomfortable. maybe they make him feel like he needs to pee? Even my son who hates transitions agrees the new ones feel better!
To be honest, if this continues for a couple of weeks more, I'd check in with the doctor, just to make sure there isn't something you missed.

I wont have much of an answer, but, one of my day care boys wet his pants ONCE.
This apparently traumatized him, because he spent the next three weeks asking me to feel his crotch. LOL "Kim, I think I peed" "Kim, check my crotch" (not what you want to hear from a 4 yr old)
Anyway, I demanded he go to the dr. They checked for a UTI, and said there was none. He got worse....I was beside myself. One day, he actually fell asleep folded up on the toilet. I demanded he go back to the DR, because surely this was not normal. Still, they found no problem.
It stopped as suddenly as it started. He has new issues now. (his freckles are dissappearing!!!) But, the "I think I might have peed" obsession did go away on it's own.
But, don't assume it isn't a UTI. It still could be.

My almost five year is constantly changing his underwear for just one drip. He has a little drip after he goes and it drives him crazy if he thinks he got a drip in his pants. I finally had his dad tell him it is normal for boys to have a little drip sometimes. It took a little time, but he is getting less paranoid.
