NH exemption letter needed asap!

someone here sent me a letter but my computer had a lot of problems and i never backed it up.
i just realized that i may need this, as my 15 yo is beginning HS and needs a physical. im worried if they say he needs something and i am NOT going to have any more vax's put into his body, period. he's fully vaxed up till now.

Hang on - there may be a preprinted form. I'll go check.

You're wise to submit an exemption now because you never know what they're going to add from now until he graduates.

thanks LI! there is a grandma on there that has a daughter in NH that has used her own and its awsesome. unfortunately, i dont have it anymore. i was hoping to "copy" that one!

Is this form fits also for NY? And what kind of lower can help me with notarization?

When we were in NH we got a form from our doctor. It was really basic and almost exactly what I had planned to make at home anyway, nothing bad like i hear some places try to make you sign. Just the applicable state law, and underneath a place to sign saying basically 'I (parent) choose not to vaccinate(child) due to our religious beliefs.' And they had a notary in the building so we had that done right there.

This is the form specific to New Hampshire. Each state has different immunization requirements and exemption laws.

New York state does not have an official exemption form. NY offers schools guidelines and an optional exemption "request" form, but there is no exemption form.

I'll get you a link, but you won't need it for NYC.

The NYC Board of Ed requires an explanation regardless. The form is just a bunch of lines that give you room to write in an explanation of your religious beliefs, which you have to do anyway for NYC. I don't even know if the NYC Board of Ed has adopted the new guidelines. The form is not an official form for the state, it's just a form used in order to guide schools if they want to use it.
