What's causing this?

Wednesday night, I nursed my daughter back to sleep lying down, and transferred her back into her cot (she's sleeping better without me atm.) I noticed a hard lump, like a plugged duct, at 12 o'clock on the boob she was nursing from, and my half-asleep brain went "ooh, that's odd." Then we all went back to sleep.
Thursday and Friday, the lump has cleared but the nipple is agonisingly painful- it hurts all the time, and nursing is excrutiating. No change in colour, size or shape, it looks the same, just hurts.
Is this teething related? Or is this something else?
BTW, she's having difficulty nursing on that side- there's a big wet patch underneath her whenever she nurses, so a lot of the milk is dribbling out. She's just cut her first two top teeth and we think she's working on a top incisor, if that makes any difference.
Also, what do I do to fix it?

It could be that the plugged duct is in the nipple area now. I had one do that. It was a few days before I could see it on the nipple. OUCH.
