My eldest was delayed vaxed, after already having his 2mo, 4mo and 6mo shots and finally selectively caught up at age 2, and my middle child only had had her 2mo shots until we selectively caught her up around 18 months. To answer your questions...
1. No, you don't have to start over.
2. I think when we were doing single vaxes (to rule out reactions) in my middle child, we had her in once a month for a shot. This did take a while to catch her up, but I wanted to be sure. There are certain time frames to wait between doses. Seems like most are a month, but it depends what it is. She had had DTaP and had reacted to the aP portion, so when we caught her up she only had DT (and at boosters also). It depends on what the shot is as far as the time between, but we sat down and mapped out how to space them out enough to rule out any reactions, yet get her caught up. (For instance, we did M, M, and R separately, although I hear those aren't as available now as they were in 2000, when we did it.)
3. I don't think age matters as much as if she is in daycare, or around sick people in general. Also, with winter coming up, people get sick more often because they are indoors and together more. I don't think the vaxes are less important. Even at nearly 7, now, I would not want my daughter to get pertussis! She's a small thing! You can selectively vax on your own schedule (according to the spacing between doses that your doctor can better inform you about the specifics). If it were me, I'd do it as soon as you can, just because winter IS approaching, but dote on her if needed the day of the vaxes. I always did with my kids when they were younger. Even when they go for their flu shots (usually around their yearly check-ups because of Fall bdays), I will get them something special after the appointment. (I also get a flu shot at the same time, as I'm asthmatic.)
