exemption question

My ds attends daycare. We selectively and delay vacc. So, I turned in a record of the vaccines he's had so far when he first started daycare, in August. Well, the health dept. was there about a month ago and said they didn't have a record of the vacc. for ds. I told the dcp we gave them a list of the vacc. and they said we had to sign the exempt form. We did, and I didn't know that we had to list the vaccines we were delaying as well. So, they called us back to list those vaccines that weren't complete. Now, do I have to give them an updated list from my doctor when we get more vaccines or do I just leave it?

Any vaccines/doses which are not up-to-date according to your state law will need to be included in the exemption. You are not obligated to give them the updated information if you have already exempted your child from receiving those particular vaccines/doses, regardless of whether they will be administered at a later date since your child is legally exempt.

Thanks. We're in CO, so I was able to do the personal reasons exemption.
