Yet another red, swollen foreskin post...

DS2 is 20 months and intact. This morning I noticed his forskin looked swollen and the tip a bit red but not too bad. It has worsened through the day - more swelling and redder - and he is now crying when he pees. He also has had some discharge - white, creamy stuff that I assume is smegma as well as a bit of more runny stuff that is not full blown pus, but not clear like urine. We did a long soak in the bath tonight and let him run around diaperless afterward. I also put a bit of antifungal cream on the tip before bed.

He has had some diaper rash lately, so I'm concerned about yeast. Could the white stuff be from yeast or could it just be separation trauma?

Also, he was with a sitter for NYE. Not our regular one and we realized later that neither DH or I gave the "don't retract" speech. (no flames please it has been a long time since we went out). Everything appeared normal afterward and this didn't start until 36+ hours later. Could it still be from the sitter retracting? (I don't think he was retracted, but can't say 100%) If so, what to do?

If it is still bad in the morning I will probably take him to the ped. Any advice (beyond don't retract)?

It definitely sounds like an infection. Can you contact the sitter to ask if she retracted him?

PS. It sounds like he will need either a topical or internal antibiotic.

I am back and forth on contacting the sitter. It was a shared sitter with relatives that we went to dinner with. Someone that they trust completely, but I don't have a direct contact for and will probably never use again (or will be super clear with if we do). I know I should clarify with the sitter anyway, but having to explain it all to relatives who think we are whacko for not circing in the first place is not something I want to get into right now. At this point it seems like the damage is done, if it was, so I'd rather not go there.

Would any damage from retraction not show up right away?

It really look normal that night (we were home before bedtime) and all day the next day - just the ongoing diaper rash on his bottom and scrotum. It wasn't until this morning (two days after) that anything looked out of the ordinary.

Anyway, is the treatment any different for an infection due to retraction?

This is a good question. Keep in mind though, most MD's still think forced retraction is ok. I can't imagine what advice they will give if they think nothing about it to begin with.

Sometimes it takes a few days for visible trauma and infection to show up. I am not sure what treatment is recommended for this situation, but I think that you need to get advice from a Pedi who is pro-intact. maybe in the next few hours one of the nurses in here will respond to this.

If he was retracted it would be a good idea for you to find out just in case he has long term issues from it (not likely but possible). If he isnt better by tomarrow I would take him in and have a culture done to see exactly what the pathagin is causing the problem.

Well things are looking better this morning! Yay!

It is still a bit swollen, though almost not enough to say for sure. He is not crying when he pees anymore and it is not as red. No obvious discharge either. DH wants me to take him to the ped today, but I am thinking that I should wait another day since it looks so much better. We are going diaperless as much as possible and I'll make sure to get another long soak in the bath tonight as that seemed to make a world of difference. I don't have any TTO, but I do have Grapefruit Seed Extract - would that be OK to add a few drops in the bath?

Given the smegma that came out, I'm wondering if it could just be some natural separation, though I can't rule out retraction DS1 is not retractable at all so I have no idea what to expect.

It could definaly be a bit of natural retraction. If that is the case then within 24-48 hours it usually looks better. I am glad things have calmed some now.

It doesn't sound like he was prematurely retracted to me -- if he had been you would have seen the redness and trauma right away, not delayed. I think the timing is just coincidental, and he's just undergoing some normal separation.

I also don't think it sounds like an infection -- white discharge usually doesn't mean infection. If it were green or yellow, really stinky, or he had a fever and it was super sore, then I would suspect infection.
