You can wear whatever you want. A lot of people will wear a bikini top/sports bra, but most that I know wear nothing on the bottom, and a lot are totally naked.
Yup, on our hospital tour, they said "Naked mamas are allowed...but no naked dads!" So DH is bringing a swimsuit in case I want him to get in with me for support/massage. I do plan on wearing a black nursing bra through most of labor, but probably nothing on the bottom...
All I wanted to add is be prepared to feel differently during labor than you expect... I couldn't stand the feeling of any clothes when I was in labor, so I'd have been naked no matter what, which wasn't what I expected, and I know some moms don't like feeling that exposed. So I'd have a couple of plan B's
I was naked last time, but may wear a tank top this time around (last birth was Sept and it was a warm night) I am afraid I will be a bit chilly this time around, I can always take it off if I want. Last labor I just didnt want anything touching me, not even clothing.
I would bring something (that if it gets stained with bloody baby body won't make you weep) to wear on the top half and have it there for an option.
I stripped naked as soon as I hit the birth center when I labored with my first and anticipate I'll do the same thing this time around. Only because I just felt better without clothes on.
Will bring swimtrunks for dh this time so he can get in the tub with me, forgot those last time around.
I made a little wrap outfit for my first birth. A mini-sarong type skirt and a strapless wrap top that tied in the front for easy access and quick on-off, if necessary. It was awesome for #1, #2 came so fast that I only got it half way on... it is ready for #3.
I'm personally not comfortable being naked and have found that to hold true in my last two labors so I do wear a bathing suit top whether it be a bikini or tankini. I don't wear bottoms, usually and when I get out I put on a swim skirt or skirt of some sort and underwear just because I'm a big walker and with the last two kept bleeding until my water broke and then it was time to push for me.
I thought I'd wear a sports bra the first time but couldn't wait to get it off but still didn't feel comfortable the nurse suggested nothing on bottom and a hospital gown on top. It's a little weird to have the fabric floating around in the jets and all and once you're out of the tub can't wait to rip off the soggy gown but the nurses had a nice fresh dry one waiting so that worked for me with #2 and 3. Didn't do the tub with #1 as we were at 10 and ready to push when we arrived. With #4 due in 2 weeks I'm bringing very little with me to the hospital. Basically just going home clothes. Why not leave the mess and laundry behind as much as possible?
I end up nude even when I don't get in the tub
Those gowns are stupid and annoying, and contractions somehow seem to make anything on me just irritating. I'm a naked birthing mama, and I don't care how many hospital people see me!

ROFL... yup, I was at a birthing center in rural Tennessee for DD's birth, and apparently I almost wandered into the hallway nakey (which in rural Tennessee would have caused quite the stir). I was oblivious, I just couldn't stand any clothes. One of the joys of giving birth at home this time is that no-one will care .
