I just came to a realization about something. My supply is low, its something I have known is a very real risk since my dd has a number of feeding issues one of the most important is she can't suck. She's managed to nurse this long because of several factors, I have a very strong let down so for much of the feed she just needs to swallow and once it slows down she's basically just compressing the breast with her jaw and tongue to squirt out the milk. She's only wetting about 3 diapers a day, she's got extremely slow growth and she's not meeting her milestones. She's 9.5mo and its just in the last 2 weeks she started rolling well and sitting for a few minutes without support. (we will be getting services but the evals and stuff just started 2 weeks ago) That said.

We've seen the LC (yes a real one and she's awesome) and she agrees dd is not getting enough and needs to be supped. Based on multiple pre and post feed weighs she's only getting 2oz every 2 hours. That's only 24oz per day and she needs about 37oz. Today I started formula because she needs food NOW.

Here's my quandary, pumping alone has never been though to get my supply up (almost 6 years of pumping here talking) but if I add in Mothers Milk Tea I can double it in about 3 days. Problem is it sets off dd's reflux. It's not projectile but she does get "spitty" and gassy and fussy and is just generally not a happy camper.

My thought is just medicate her though it and put up with it and start pumping to add in the stimulation to keep up the increased supply.

Increasing her solids is not an option, I was actually told by OT to stop them and I'm only to feed her purees with this tiny little dropper they gave me to use along her cheek. Something about aspirating and not being able to handle the solids I thought she was doing great on (although we immediately ran into constipation due to lack of fluids)

So, wwyd? Put up with up to a week of misery to get things back on track or just skip the MMT and pray pumping alone works? I already eat oatmeal and drink a ton of water and I'm allergic to Reglan so that out too)

Hopefully someone will post with additional galactagogues to suggest to you. I used oatmeal, MMT, and pumping to up my supply (plus trying to get enough sleep etc.).

If you can find something other than MMT to try, I'd try that first in hopes that you don't have to put yourself and your little one through the MMT.

how is she handling the formula?

personally, *if* she's handling the formula well, I would not make her more miserable by adding in the MMT, I would just pump. Have you ever considered using Reglan/domperidone instead of MMT to build your supply? you sound like a great candidate for domperidone...

but I cannot stand to watch my daughter reflux (hers has been really bad lately so that's my bias there.

She does fine on the formula but we do have a very limited supply of it. As I pointed out I'm allergic to Reglan and last I heard customs was confiscating all dom shipments into the country. I just hate having to give her formula, its not the formula itself but the time required to feed her since she can't use a bottle and I have to cup/spoon feed her.

Have you tried something like Motherlove's More Milk Plus tincture? It works great for us. My DD also has reflux and we chose to medicate her. It was a hard decision but it was the best one for our family.

I almost wonder if your DD's reflux is aggravated just by the fact that she's actually eating enough, and isn't used to that. Can you try to increase your supply, but feed less at a time and more frequently? Perhaps her tummy is just very small from everything she is dealing with and she can't handle the usual amount of milk at once.

Good luck, it sounds like you are dealing with a lot. Kudos to you.

oh, I'm sorry, I missed that detail. Cup/spoon feeding would get really old, I can understand that! We have to supplement with formula also (I have low supply), but thankfully she'll take a bottle

I didn't know that about domperidone, and I was about to order some...hmm, I'll have to look into that.

Anyway, good luck. I second the idea that maybe her tummy is just so small that an increase in your supply throws her off. I hope you figure out something that works for you. Reflux is hard.

Can she not use the bottle b/c of the sucking? Has the LC mentioned a Haberman feeder at all to teach her how to suck. We are going through this exact same thing except Ilana is 4 mos old. I see a bfing DR (we're lucky to have one of these) on Monday to get a full eval from her. We started supplementing 3-4x a day last Friday and Ilana has already gained weight and is much more alert during the day and awake and laughing etc. We are using the playtex slow flow nipples usually and 1 stage 2 Avent which doesn't make her suck hardly at all, it's so fast! I'm also pumping w/ the lactina after almost every feed and taking brewer's yeast and oatmeal. I can't do fenugreek and blessed thistle is probably out b/c of my allergies and Ilana's chance of having seasonal allergies as well (she's already milk, soy, egg intolerant and both boys have seasonal allergies too). I can't stand MMT. I think my supply is coming up as I don't need to supplement as much now, but I can also pump an oz or so after she's done eating so I know she's not fully draining what's in there either. Ilana also has reflux so that's never any fun. I'm not sure any of this helped, but I wanted you know someone out there knows exactly how you feel and if it were me, I'd skip the MMT and try something else.

we used the MMplus tincture and it worked really well for me.

The thing with reflux is: it's so random! You can think it's the suppliment setting it off, but it could just as easily be the weather unless you've been through it several times.

I'm read that supplementing milk causes a foremilk/hindmilk imbalace and increases the watery formilk...I still think this is preferable to formula. I'd rather give solids then use formula.

We used everything..including formula (which I HATED with a passion) and all it did was cause stinky vomit and lingering allergies.

If I had to do it over again (and it was the hardest, most stressful time in my life..though I didn't know it then it was simply living from one feeding to the next!) I would have plugged my ears, stayed away from drs and nursed as he wanted. I did eventually do this and things improved slightly.

We used slow flow nipples the entire time. They are a must! I also recommend if you are nursing to use the breast that has the slower flow. I have one formerly inverted nipple and he spit less when using that one. If you are bottle feeding or supplemental feeding (EBM in another form) I'd collect and four off the thinner lower calorie milk in order to get him to gain. (we were barely gaining an oz a month, he was the size of a 4mo at 10months and we typically have giant babies.)

I used to joke about sewing coins into his diapers for the HATED weigh ins every 7 days.

I would try the domperidone. Where did you hear about the seizing all shipments at the border? Is that a new thing? Last I heard it was just a random, rare event :

I thought thats what I heard on the pumpmoms list a while back. Where can I order it?
