bleeding is bothering me

My baby is 6 weeks old, and we got off to a rocky start (he was on a ventilator for awhile, didn't start nursing till 2 weeks old, and instincts were definitely tampered with). I have a good supply. I thought we finally had a correct latch about a week ago, and things have been relatively comfortable, but now my nipples are bleeding at the beginning of every nursing session. This is the first time I've noticed it. I probably wouldn't notice except for the fact that we're using a nipple shield, and I can take him off the breast and see the red blood inside the shield. After a minute or two it's just my creamy milk. It really disturbs me. I've heard it's okay for him to ingest it, but it's hard to believe.

Do you think the nipple shield is adding to the blood? I think the bleeding cracks run in a semi-circle around the nipple. I tried without the shield: a) he doesn't like it; b) it made my nipples hurt WAY more!!

I've seen a LC twice and talked to a LLL once. I coat my nipples in lanolin from time to time. I think we're getting the hang of the latch, but probably not. I take ibuprofin when it's really bad, which is huge for me since I believe pain is a sign something's wrong and I usually try to get rid of pain in another manner. I can see why people quit bf, and if I weren't so determined, I probably would, too.

Advice?

Oh mama, what a lucky baby you have to stick with nursing! You are working so hard!

The blood that you see is okay for him to ingest. If you are having pain though, you need to talk/see someone about it. It is not supposed to hurt and your nipples are not supposed to bleed, even with a nipple shield. A La Leche League Leader can help you with this and has an information sheet specifically about using nipple shields.

Using the lanolin here and there is okay, using it after each feeding will help them to heal more quickly. YOu can also express some milk (or use what's in the shield) to rub into your nipples/areola and let air dry. Your milk has healing properties and will help them to heal. So, you could use your milk, then the lanolin. And remember that the lanolin does not need to come off before nursing your son. Avoid using anything like soap, detergents, etc. on your breasts/nipples/areolas, especially while they're sore. That will only make it worse. All they need is clean water.

When using a nipple shield, remember to keep a close eye on his wet & poopy diapers. He shoudl have at least 5-6 really wet & 3-4 quarter size or more poops a day.

You are doing a wonderful job contiuing to nurse your son. It is often difficult in the beginning, even when a baby latches on perfectly from the first moments after birth. The two of you have it many speed bumps - you can do it!

Get IRL help - you and your son are working so hard - don't do it alone!

thank you for the encouragement. I'm feeling a lot better. Yesterday he ate a LOT and somehow that helped--maybe since my breasts weren't as full.

He's started to only poop once a day. He completely fills his diaper, usually gets his clothes, but there's only been one. Is that normal? six weeks old. Also, the color has changed within the last 4-5 days from mustard yellow to a lightish brown.

Hi again.

So glad to hear you're feeling better!

Yes, it's normal for a baby to poop once a day, escpecially since he's pooping so much. The color isn't typical but I'm wondering if it's because of the blood from your milk.

I'd still suggest you talk to someone in real life about this. A reputible LC or a LLL Leader.

Keep nursing and loving your babe!
