GTT and Metformin

Hi there..

I have an appt with one of the midwives this morning and they are supposed to do the GTT test..I am just eating a normal breakfast and going in for my appt when it's time....it's at 11. They are only asking that I take it bc dd was 8 pounds 15 ounces when she was born 3 years ago....I was really sick the first part of pregnancy with her and lost a good deal of weight, gained back 12 according to the scale and she was still "big"....My MW had no problem with it. The OB's in the practice did but whatever..they aren't my provider...One consults with her bc of my history but it's not a big deal..she's great and more like a MW than an OB.

I do have a question for my friend though...

She is supposed to have her GTT later this afternoon and is taking 1000mg of metformin/glucophage bc of her PCOS and doesn't think she should have to drink the syrup like crap bc she's already taking he meds that help regulate her insulin resistance and she would get a bunk reading anyways...so what's the point. Her OB is insisting on it and she's not one to argue....I told her to just eat normally and then go in and tell them "yeah, I drank it at 2:15" and she's thinking about it....She normally takes 500mg of the Metformin at noon (did so with her first pregnancy too) and thinks that the Glucola will make her sick bc of all the sugar mixing with the meds that are supposed to help with sugar.....So, I told her I would come here and ask for advice.

Her first child ended in stillbirth at 27wks....and was "labeled LGA for large for gestational age..." Her second was born a day shy of 42wks and was 9 pounds 2 ounces and this one is "measuring" 3 weeks ahead consistantly even though they only BD'd once in December so the date of conception is pretty dead on.... I think she just had big babies...nobody else in her family does but she does and births them just fine....last one with an epidural bc the back labor was horrendous she said...

Any advice for her regarding the GTT and Metformin...I know the popular belief here is it'snot needed and I tend to agree esp in my case this go round bc I think I would vomit it right back up..I'm just hitting the good part of not being consumed by Hyperemesis and don't want to mess with it......but she may just take it as to not cause waves...

She tried calling the office yesterday but even though they are there until 6pm the phones go to machine at 5 bc the pateints are gone for the day....her appt is today and the office doesn't open until 11..

I had PCOS as well, and took Met through both of my pregnancies. Maybe as an alternative, she could offer to test her blood sugar for a few days and record the results? She could certainly still develop GD even with the Met, but it's less likely, especially if she's eating well. The more important issue (rather than how her blood sugar responds to an artificially high amount of sugar) is that her blood sugar is under control day-to-day, which it likely will be if she's eating well and exercising.

About the necessity of a GD test overall... I'd refer her to the great info in "The Thinking Woman's Guide to a Better Birth."

However, I would encourage her NOT to lie to her care provider. I don't think that's ever a good idea! They deserve to have full and accurate information about her health status. It's important to have clear and honest communication, and to be straightforward that she's making an informed choice if she disagrees.

yeah, I think that being on Metformin doing the GTT would totally be inaccurate -

I'm not a fan of the GTT, but random blood sugar checks. Chances are if she's on Metformin, she's already doing that.

she's not reguarly checking her levels bc shes on metformin...just on a standard dose for the pcos...there is no diabetes in her family....

she went ahead and drank it and within 10 minutes her hands were shaking big time and she threw it up....she wasn't happy...neither was her OB but she told them it would happen..she doesn't normally eat that much sugar in like a week so.....she also said that she wasn't going to reschedule the appt bc her glucose levels had been tested 2 months prior after just eating a normal breakfast with her endo (we see the same one) and her levels were well within the normal range....

i think she may be switching practices if her insurance covers the MW she wants to use now....

not a happy camper...my appt went well..lost a pound but they arent concerned....and MW was ok with me just eating an ormal breakfast...said she expected it...lol
