Tricks for persistent OP?

When the baby isn't coming down with pushing and/or there's still a persistent lip, how do you manage? I've read about the hand maneuvers in Heart and Hands, but I'd also like to here some experiences -- do you use hand maneuvers? If so, how exactly? Have you ever/often had to transfer a woman with no problems except persistent OP baby?

TIA for any info you wise womens can provide.

I don't do a lot of vaginal exams so don't find many lips!
If the labor seems hung up or the baby in a less than good position, try the rebozo. It works very well. Shake and jiggle that baby around.

I've gotten clients to spin their OP babies late in labor with the side lunges with raised legs, belly lifts and pelvic rocks. Used hands and knees poition with pelvic rocks to address any cervical lips. Moving the mama's position to place head pressure with gravity on the remaining cervix seems to help (not all the time, but some).
