I did, but they made a mistake. I filed a claim for reimbursement and they sent a check for most of it. When I called to ask why they didn'y cover my stitches (the MWs are "out of network"...do they expect a perfectly healthy woman who just gave birth in her bedroom to drive to the hospital at 2:00 am? My regular doctor was appalled and sent the insurance company an angry letter.), they looked at my stuff and said "Oops! We weren't supposed to cover that!" But they just let it go.
They had told me before the birth that they do cover prenatal care with a midwife. If you can get your MW to write up your bill as being all for prenatal, you might get something back. Also, I guess part of the issue with my claim was the fact that my MWs are not CNMs.

Anthem Healthkeepers BC/BS. My policy book says that they cover homebirth. My midwife says that they typically cover all but about $200.

I have BC/BS of Oregon (never heard of the "Anthem" part...?), and they covered our prenatal care and most of the homebirth attempt. We ended up transferring and having a c/s, so that caused a little confusion, but it worked out OK.
They are also covering 70% of our planned birth center birth (essentially a homebirth somewhere nicer than my house). I think they would cover more, but this birth center isn't on their preferred provider list.
