I love the document sharing! How bout med history/initial meeting forms?

I have one i made that just isn't the best and one I copied from a book but i can't remember which one, anyone have one they would share for ideas?

thanks for sharing! I don't have mine on the computer, but it needs to be updated anyway. I just bought all the forms from maws (washington midwives, I forget this morning without my coffee yet what the initials stand for). They are color-coded, which is a cool idea.

Anyone else color code everything? I'm thinking I really should do that and follow through with my birth bag

Ok, first off you are assuming that because I'm obsessive about my forms that I'm somehow organized. Any of my apprentices could tell you differently!

The MAWS charts had information that I didn't need, but the biggest thing was that those darn spaces to write in were TOO SMALL! It drove me crazy because I'm a big writer and hate to be confined to small spaces. So I started creating my own forms. I change my forms alot - I"m pretty obsessive about them. Seriously.

I don't usually color code things. I used to have a red bag that contained all my hemorrhage supplies, but after my initial birth bag was stolen out of my car, I never bought a new red bag. I think cosmetic cases work really well - especially the clear ones!

I love the fact that you have everything online! I am going to be helping one of my preceptors to do that with all her info sheets and everything for organization purposes. I just think it is a great idea!!!

I do color code and I also bought the MAWS charts. That's where I got the idea. Now I have all my handouts the same color as the chart forms. I use the extra manila page in the center of the chart and really try to keep all four sections straight. It is just much easier for me to find stuff in the file drawer and the chart if I at least know the color I'm looking for.
I agree with Pam about the spaces on the MAWS charts. Although I write really small and just don't have enough space. Finally gave up and just made my own forms.
