Nursing strike!!

As of yesterday my 4.5 mo dd is doing a nursing strike and I'm feeling horrible. Of course I feel guilty for introducing formula a couple of weeks ago after she was screaming every night for more and my supply wouldn't increase after all the tricks of the trade. So now she seems hooked on the fast flow feel of the bottle. I supplement about 6-8 oz with organic formula, but I also pump and bf.
She latches on about 15 times in a minute, screams, relatches, screams, and on and on. I am just getting over my second severe trush episode where it was painful to nurse. I've only had about a weeks worth of painfree nursing so far and maybe my pain has finally affected her enough to the point where she says "screw this".
But I'm not ready to give up bf here, I really want to continue providing her with the best milk in the world.
I'm rather upset and angry at myself.

You must get rid of the bottles.

I'd recommend dropping the bottle for at least a couple days, to get her back to nursing. Also, take away the pacifier if you're using one. Try to get her to do any extra comfort sucking at the breast as well. If you can nurse laying down, take her to bed with you and nurse as often as she's willing to.

Make sure you're properly hydrated. At least during the daytime hours, drink something each time you nurse her.
