vax exemption letter

What do you think of this?

And what does having it notarized do?

I respectfully request that my child be exempt from the requirement of TCA Section 49-1767. I assert that my child is entitled to the exemption provided in TCA Section 49-176, Section 2 amended in that the required immunizations would conflict with his religious beliefs and practices. I have a strong conviction against introducing foreign bodies into my child by way of immunization and to do so would be harm to my child.

I assert that there is presently no epidemic or any immediate threat to the members of the community if this requested exemption is granted. I release any care giver for my child from any such liability due to this decision, and take full responsibility for the health of my child.

I don't know what state you are in, but I think you have too much information in there; you are "asking" for an exemption instead of "taking" one. I would go with:

"In compliance with "California" state law XYZ.123.0, I Jane Doe, parent of Jack Doe, declare that we are religiously oppose to the practice of immunization."

Notary public from Wikipedia:

Quote:
A notary public is an officer who can administer oaths and statutory declarations, witness and authenticate documents and perform certain other acts varying from jurisdiction to jurisdiction. For the purposes of authentication, most countries require commercial or personal documents which originate from or are signed in another country to be notarized before they can be used or officially recorded or before they can have any legal effect.

I agree with pp - don't ask. State you are taking an exemption for religious reasons and don't go into detail. CA also allows philosophical exemptions so don't fret about not having your religious exemption accepted. In other words, someone can write that they are taking an exemption because they simply don't believe in vaccinating and the state cannot argue. I'd go with the religious exemp though as this is less likely to ever be revoked.

ETA: sorry, I first should ask what state you are in - I assumed CA...

So I can just say I'm religiously opposed to vaccinations and don't have to give any further info? My state does not have philosophical exemption.
