I'd go for the real thing when it comes to stethoscope and reflex hammer. A real oto-ophthalmoscope would be a silly extravagance for a child that age, but a real stethoscope isn't so expensive (there are some deluxe versions but also cheapos) and can usually be adjusted to fit a child's ears. A sphygmomanometer, while fairly cheap, I'd wait a few more years for. Disposable syringes are pretty cheap and make good bath toys (squirt guns); the seals wear out or get stuck, so get a bunch, minus the needles of course. Even electronic thermometers are cheap these days, and safer than the old glass ones. If your child's safe with a lollipop, he should be OK with tongue depressors. And of course plenty of bandages, with which he'll proceed to bandage everything in sight, be warned.
For really cheap, ask your doctor. S/he may have some giveaways from drug makers s/he can pass along.
