Hi, folks!

I am really excited that I will begin facilitating my first Childbirth Preparation Circle next week. I'm doing a series of 6 - 7 meetings/classes, loosely based on BFW. I've been a professional educator for 10 years and have a life-long interest in childbirth etc. so I have lots of ideas and am so looking forward to trying them out. I was more or less asked to do this by one of the couples I'll be working with and . . . I am just so pumped :.

I need/want to create some kind of registration/intake form for my records. Does anyone have a great one they could share? Or ideas about what should go on it? Here's what I'm thinking:

Mom's name, age, marital/relational status, contact info, # of pg/# of children

Dad/partner's name, age, contact info if different

EDD, planned birth place/attendant

What else do I need/want to know?

Would it be weird to put a place at the bottom of the form labeled "Office Use Only" or something with boxes for me to check off about birth outcomes later on when I hear about them?

It's a good idea. I also ask what they're most worried about or interested in learning. I would not put "office use" at the bottom because it may make them think you're keeping "records" on them. I just keep my own personal log of outcomes.
